Proximity Wellness accepts many insurance plans. Patients should check with their insurer to determine eligibility and co-pay requirements.
The following fees apply to patients who prefer to pay out of pocket for their treatments.

Assessment fees are charged separately from the cost of medication. Our patients can use their insurance if they wish to have prescriptions called to a local pharmacy.

Patients who wish to have prescriptions mailed to their homes may opt to use one of our compounding pharmacies. Compounding pharmacy fees are not marked up for our patients, and costs are passed through to the patient via PRX. Please note that lead times for the initial prescriptions filled through the compounding pharmacy are 5-7 days. ​

All self-pay plans incur a $75.00 initial consultation fee.

Consultation fees are due prior to the first visit.
